Getting the most from your surgery

GP surgeries have changed the way they work to meet patient needs and increased demand. There is now a wider range of medical staff at surgeries and different ways to get help such as telephone, video, and online consultations, as well as face-to-face appointments.

Access to GP services is an issue we hear about regularly at Healthwatch Northumberland. In our ‘getting the most from your GP surgery’ booklet we explain more about the range of staff roles at surgeries, different ways to access healthcare and how to make the most of your GP appointment.

Why do I have to tell the receptionist so much about my concern?

Reception staff need to ask enough questions to make sure you get the best appointment with the right person. This may not always be your GP, as there are specialist services that may be more appropriate for your needs, for example, a dietician for
concerns around food allergies or intolerances.

Who might I see at the GP surgery?

Your GP surgery will employ some or all of the healthcare professionals listed below.

Advanced Practitioners: include nurses, pharmacists, paramedics, physiotherapists, occupational therapists, dieticians or podiatrists. They can help with a variety of health conditions and can prescribe medication and make referrals to other specialist services.

Clinical Pharmacists: can carry out medication reviews, particularly for long-term and chronic health conditions. Some can also prescribe medication and manage prescriptions.

Care Coordinators: ensure patients have more joined-up care and that their care needs are met, particularly for those who are frail and/or elderly, and people with long-term health conditions

Dieticians: diagnose and treat dietary and nutritional problems, give advice on food and nutrition, and offer support with diabetes, digestive issues, food allergies and intolerances and weight loss/gain.

General Practice Nurses: can do many of the same tasks as GPs. They assess and treat people of all ages, provide wound care, screenings and blood tests, as well as vaccinations. They also offer advice on contraception, women and men’s health issues, weight loss and stopping smoking.

Health and Wellness Coaches: find solutions or lifestyle changes to enable people to lead happier lives. They help people to form a personalised health and care plan to achieve their goals.

Mental Health Practitioners: support people with severe mental health illness to live well in their communities. They can work with patients whose mental health needs cannot be met through, for example, talking therapies, but who may not need care from more specialist services such as psychiatric care. They can often help children and young people as well as adults and can help make referrals to suitable services for support.

Occupational Therapists: work with ill, disabled or injured patients,including those who have returned home following a hospital stay. They help with adaptations to people’s homes so they can continue to live independently.

Physician Associates: work alongside GPs. They cannot prescribe medication but can prepare prescriptions for GPs to sign and can diagnose, order tests and make referrals.

Physiotherapists: work with patients who have joint or muscle problems including those recovering from serious injuries or illness and those with new injuries. They can book scans and tests and are trained to recognise when a joint or muscle problem may be a sign of something more serious. Patients can often make an appointment with a physiotherapist directly themselves (self-refer) or can be referred by a GP or other healthcare professional.

Paramedics: can provide a rapid response to patients with long-term conditions, minor injuries and minor illness. They can supply a range of medicines and support patients who require wound care, have fallen, or have musculoskeletal problems, and can treat some types of infections.

Podiatrists: can diagnose and treat foot and lower leg conditions and provide foot care for short-term or long-term conditions.

Social Prescribing Link Workers: help address the non-medical issues that may be affecting your health and wellbeing. They can connect you to local services and community groups for practical or emotional support.

Following a public meeting at Felton Village Hall on Tuesday 31 January regarding the development of the new Felton GP surgery, further responses have been added to the Frequently Asked Questions on the project website. These new FAQs address some of the questions raised in the survey responses with answers from the developer, Assura.

Questions include car parking, emergency access for ambulances, when the building will be complete and the space available in the new surgery.

Proposed changes at Felton and Widdrington surgeries

Changes are planned at the Felton and Widdrington surgeries and patients have 12 weeks to give their views and ideas on the proposals. If you are a patient of either surgery you should have received a letter and an information sheet explaining the proposals and how you can share your views.

Healthwatch Northumberland is supporting the surgeries by holding two online engagement sessions via Zoom for patients who would like to discuss any concerns or ask a question.

The sessions will take place on Wednesday 9 June at 2.00pm-3.00pm and Thursday 17 June at 6.00pm-7.00pm.

This is an opportunity to give your views on the following:

  1. The proposal to merge both Felton and Widdrington surgeries
  2. The practice’s plan for a new surgery in Felton and the permanent relocation to these premises at a later date
  3. The potential temporary relocation to Widdrington Surgery should the practice need to move out of Felton, before the new build is complete.

If you can’t make these sessions there are other ways you can get in touch:

  1. Write to the practice manager and mark it ‘Engagement Feedback’ – the address can be found at the top of your letter.
  2. Contact the practice manager by email at Felton.Surgery@nhs.net or Widdrington.Surgery@nhs.net with the title ‘Engagement Feedback’
  3. Ask the reception team at either practice for a feedback form. This can be filled out anonymously if you wish. You do not need to give any personal information if you choose not to.
  4. Contact Healthwatch Northumberland, the statutory independent champion for patients on 03332 408 468 (local call rate) or email info@healthwatchnorthumberland.co.uk.

The surgery will write a report based on this feedback in the late summer. The report will form part of the formal application to the Primary Care Commissioning Committee which will meet in August to decide if the proposals can go ahead.

How to contact your GP and get the help you need

As a result of Covid-19, the way patients access GP practices has had to change. Whilst this can be frustrating and unsettling at times, it is necessary to make sure that all staff and patients are kept safe. The following guidance from Northumberland Clinical Commissioning Group will help you to contact your GP and get the help you need.

What to do if you need to contact your GP

If your needs are non-urgent, adult patients can contact their GP via the e-consult service on your practice’s website in the first instance. You can contact your GP practice and get a reply within two working days. You can also request medication, sick notes and bloods results through this service.

For health care needs regarding children, urgent problems or for those patients who are unable to access online services, you should telephone your practice and your call will be answered as soon as possible. The practice may ask for a brief summary of the problem to help direct you to the most appropriate member of the GP practice team.

Please note that your consultation may not need to be with a GP. It may be that the care/advice you need can be given by another member of the practice team.

Where possible and appropriate, consultations will be conducted either online or via telephone. If a face-to-face appointment is required and you need to visit the practice, please wear a face covering (unless exempt) when you arrive.

Face-to-face appointments

Your temperature may be taken on arrival and you will also need to sanitise your hands. Please be aware that the surgery doors may be locked and you will need to be granted access by a member of the practice team. There will be less people in the waiting room than usual and chairs are positioned apart from each other to ensure you can observe social distancing.

Please ensure you observe social distancing while in your GP surgery. There is clear signage to guide you. You are asked to wear a face covering if you need to attend the surgery in person.

If you have any coronavirus symptoms including a high temperature, new, continuous cough or a loss or change to your sense of smell or taste please DO NOT visit a GP surgery. You must stay at home along with anyone you live with and anyone in your support bubble. Only leave home to get a test. Self-isolate until you get your result.

Urgent medical needs

Please don’t delay in contacting your GP practice if you have an urgent medical need. The NHS is here for you and your family.
You can also get urgent medical advice online from 111.nhs.uk or by calling 111. For life-threatening emergencies please call 999 or go straight to A&E.

What practices are doing to ensure services can be resumed as soon as possible

Practices are working extremely hard to manage the backlog of work and patient appointments as a result of Covid-19. Please be assured that they will be doing all they can to arrange appointments and services for you as quickly as possible.

Please also be mindful that, at times, workforces may be affected by Covid-19, so teams may be smaller than usual. They are, however, all doing their very best to look after you. We ask you to be patient, polite and respectful. No type of abuse will be tolerated by any member of staff.

All the necessary steps are being taken to protect you and staff from coronavirus. Robust measures are in place to ensure that practices are safe.

When to self-isolate

If you have symptoms of COVID-19, however mild, you must self-isolate for at least 10 days from when your symptoms started. Please do not visit your GP surgery. You should arrange to have a test to see if you have COVID-19.

If you are not experiencing symptoms but have tested positive for COVID-19, you also must self-isolate for at least 10 days, starting from the day the test was taken.

If you develop symptoms during this isolation period, you must restart your 10-day isolation from the day you develop symptoms. If you live with others, all other household members must stay at home and not leave the house for 14 days. The 14-day period starts from the day when the first person in the household became ill or if they do not have symptoms, from the day their test was taken.

High risk groups

If you are in a high risk group, you can still go out provided you are well but you should:

  • Maintain two metres distance from people outside your support bubble
  • Avoid crowded spaces
  • Wear a face covering in shops and other enclosed spaces
  • Avoid touching your face
  • Wash your hands carefully and more frequently than normal
  • Use sanitiser outside your home (where available) especially as you enter a building and after you have had contact with surfaces.
