Things I wish I had known before my loved one went to live in a care home

Hearing feedback from relatives of those in care homes, we know that the process of moving into a care home can be a very isolating and upsetting time. There are many different emotional and practical aspects to consider.

Sometimes decisions and choices have to be made very quickly. Even when there is more time to plan, there can be smaller, unexpected issues that arise and make you think ‘I wish I had known this before my loved one went to live in a care home’.

This list was put together by people who attended our care homes forums in 2021 and 2022. It is a simple list of things to think about and comes from their experiences of having a loved one living in a care home.

There will be other things which we have not thought of and if you have more ideas and tips, please let us know and we will add them in.

Things to do

  • Provide more socks than you think will be needed – with recognisable patterns
  • Provide slippers – two pairs at least
  • Keep a supply of clothes at your home to top up/replace items as needed
  • Label everything – clothes and other items – with your loved one’s name. They may change rooms but not their name!
    – iron in labels for clothes and shoes in addition to the home’s own labelling
    – use indelible marker to write name or room number on TV remotes and ornaments
  • Check if you can you use your own bed linen and/or curtains
  • Ask if you can move in small items of furniture. A small side table that sits against a wall gives a nice focal point for ‘knick knacks’ without taking up space
  • Think about things that brighten a room immediately – pictures, trinkets, lamps, throws, cushions, but nothing too valuable or fragile in case of breakages/loss
  • Put a favourite picture of your loved one when they were younger in a prominent place
  • Buy a spare remote control for the TV
  • Think about a small Christmas tree and decorations. Ask family and friends to send cards directly to the person in the home so they feel part of the celebrations.
  • Supply games, picture books etc. to help with visits
  • Take scented drawer liners which add a nice touch to a room

Things to know

  • Labelling is very important but things will disappear, move around the home and reappear – both in and out of your loved ones room
  • Remember to say hello to other residents – you are in their home
  • Care home tea is usually very milky!
  • Care homes can be very warm – wear layers
  • Are meal times protected – is visiting not allowed at this time or can you visit to help your loved one eat?
  • Find out the arrangements for Christmas – are you able to visit or take your loved one out of the home?
  • Leaving your coat and handbag in the car or in the manager’s office, so that you can slip out, can make leaving after a visit less stressful for you and your loved one.
  • Does the home arrange ‘virtual’ visits – who does them and how is your loved one supported to interact with the call?
  • It might be reassuring to ask what a ‘typical day’ looks like for your loved one
  • Visiting can be upsetting so it can be an idea to have someone to talk to afterwards or have a relaxing activity planned

Home care services in Northumberland

Home care services are a vital way to support people by meeting their health and social care needs in their own homes.

‘Understanding the experiences of people using home care services, then, now and in the future’ was a project designed to find out the experiences of people using services and their family carers in Northumberland. We wanted to find out what is working well for people and also make recommendations for changes or improvements to services, based on what people told us.

This is particularly important at present given the disproportionate impact of the COVID-19 pandemic on those using home care services and their families, and the need to prepare for similar situations in the future.

We also wanted to follow up and extend on our previous work which focused specifically on the Coquet Valley, by gathering the views of people throughout Northumberland. Read Home Care Provision in Coquetdale.

This report is an overview of the findings and draws out key recommendations on the issues raised by those who use services and their carers. We have indicated where the recommendations are for Northumberland County Council Adults Social Care commissioners (ASC) or service providers (SP).

‘Because We All Care’ – tell us what you think about services

Healthwatch England has launched #BecauseWeAllCare with the Care Quality Commission to help improve care by encouraging everyone to share their experiences of health and social care services.

Public feedback helps spot what is working well and what could be better. With services working to respond to the COVID-19 pandemic and deal with a backlog of care, people’s views have never been more important.

New research from the Care Quality Commission highlights the impact lockdown measures have had on the wellbeing of people who use care services:

  • Nearly three-quarters of carers (73%) say that the COVID-19 lockdown restrictions have impacted on the mental health of the person they care for.
  • Over half (56%) of carers say that the COVID-19 lockdown restrictions have impacted on the dignity and independence of the person they care for.

The research also found that throughout the pandemic, care was most commonly impacted by:

  • Increased waiting times for appointments and procedures (49%)
  • A lack of resources and equipment (24%)

With just 17% of people in England expecting services to improve in the next 12 months, we are calling for you to feedback on the services you use as a crucial way to improve their quality.

Sir Robert Francis QC, Chair of Healthwatch England, said:

“Since the outbreak of the COVID-19 pandemic, public feedback has played an important part in helping the NHS and social care services spot and respond to issues caused by the virus.

“As we continue to live with the pandemic, the NHS continues to face huge pressures across the whole system. NHS staff are grappling with many difficulties, including increased COVID demands, while trying to tackle the backlog in elective care and diagnostics that the pandemic had made so much worse.

“It is important that this is managed as well as possible, to make sure that the risks and distress to patients are minimised. That’s why it is vital people have the opportunity to share their views and experiences of care whether they have needed treatment for COVID-19 or other illnesses. Their feedback will help services understand both key national and local issues and the steps they can take to serve patients better.”

We are holding another public online forum via Zoom for anyone who would like to share their recent experiences of visiting loved ones in care homes.

The forum will take place on Monday 7 February, from 10am to 11.30am.

At our previous forums we have heard how visiting restrictions in care homes have affected people living there and their relatives, people’s experiences of other ways of keeping in touch during Covid-19 and discussed things ‘we wish we had known’ when our loved ones moved into care homes to help us produce some future guidance to support others going through similar experiences.

Those who attended the previous forums said that it helped to give them a voice and it was helpful to meet with other people in a similar situation. It was agreed that the group would continue to meet once every three months to discuss any issues or concerns. The minutes from the last meeting can be found below.

Care homes, Covid-19 and beyond

In September 2020 it was widely reported that people who were living in care homes in Northumberland during lockdowns had struggled to keep in touch with their loved ones, as visiting restrictions were in place to protect the most vulnerable from infection.

We responded by asking for feedback from people whose loved ones were in care homes in Northumberland. We hosted a series of online forums which took place in November 2020, January 2021, March 2021, and June 2021. We had 46 attendees across these four forums consisting of 26 different people, as many attended more than one and in some cases all of the forums.

At different stages of changes in visiting guidance for care homes these forums gathered feedback from relatives around their experience of visiting and other ways they were keeping in touch with loved ones during the pandemic. We also heard from professionals who work with older people including those with dementia, their carers, and other professionals in the health and social care field.

It is important to acknowledge that the Covid-19 pandemic was unprecedented and care home staff had to work under extremely difficult and challenging conditions for a long time. Whilst many relatives who attended the forums generally had less than positive experiences to share, they were also keen to know what good practice was taking place across care homes.

Visiting experiences had also improved by our final forum and survey responses have also shared positive approaches taking place. We ran a survey for relatives of those living in care homes and care home staff from mid-March to early June 2021 to gather additional feedback. The survey had a total of 22 responses.

At our previous forums we have heard how visiting restrictions in care homes have affected people living there and their relatives. We have also heard about other ways people have been keeping in touch during Covid-19 and experiences of the changes to allow one or two designated visitors for in-person visits. We have also heard from representatives at Public Health and Northumberland County Council on the visiting guidance for care homes.

At our next forum on Friday 4 June at 10.30am we’d like to hear about your experiences now we are two months into the changes in visiting policy. We would also like to hear your feedback on preparing for you and your loved ones to have visits outside of the care home.

We will also be sharing the results of our recent online surveys for care home staff and relatives/friends of those living in care homes and looking at the next steps for this feedback moving forward.

Our public forum on care home visits in November produced some really interesting and insightful discussion. We talked about how people are keeping in touch with loved ones in care homes during the pandemic, the effect on those in the homes, particularly those with dementia, and the effect on relatives, plus what would help, going forward and throughout the winter to make the situation easier.

Further to the issues raised at the forum, where people told us they were unclear on what care homes can or can’t do in terms of visits, we asked Liz Morgan, Director of Public Health in Northumberland for information. She shared the guidance on visiting that has been sent to every care home in the county, which can be found below.

Each month, thousands of people share their experiences about NHS and social care services with Healthwatches across the country. During the COVID-19 pandemic, this has ranged from the effect lockdown has had on carers, to the problems getting emergency dental treatment.

In Healthwatch England’s latest briefing they outline the issues over 19,700 people have raised, as well as taking an in-depth look at how technology has been used in response to the pandemic.

The impact of COVID-19

At the start of lockdown, people told us how the measures introduced to help control the spread of coronavirus were affecting their care.

Changes to routine and planned care – In many cases, people were unable to find the information they needed to understand what they should expect from services and were unclear about what the next steps for their treatment or care would be – leaving them feeling stressed and frustrated.

Shielding measures – Those who were shielding told us about problems in getting transport to their hospital care. For some people, the cost of attending one or more hospital appointments was too expensive to arrange private transport, especially if the hospital was far from home.

Access to prescription medicines – Initially people were struggling to get through to their GP or pharmacy by phone, and others experienced delays in getting their medication. This caused anxiety for people running out of supplies, particularly for those with long term conditions.

What can services learn?

  • People need clear, accurate and consistent information about their care and the services they use.
  • People’s experiences of hospital appointments do not start and end at the hospital doors – their journey begins at home, so transport arrangements must be considered.

Lockdown begins to ease

As lockdown restrictions began to reduce, we started to hear new concerns from people.

  • Worries about the future – People raised questions with us about how services can reopen safely, reported problems using services that are supposedly already open for business and expressed frustration at some NHS services being slow to reopen compared to other areas of the economy.
  • Testing for COVID-19 – While some people found visiting a testing centre easy, we also heard that the online booking process was difficult to use and there were concerns about the accessibility of testing centres.

What have people been telling us throughout?

  • Lack of accessible information – Throughout the pandemic, we have heard about the difficulties of finding up-to-date information in the languages or formats people need – especially when advice from the Government was frequently changing.
  • Emergency dental care – People did not know how to access emergency dental care – causing them extra stress while experiencing acute dental pain or other symptoms. Many others have felt they have no option but to go private if they want to receive treatment for what their dentist considered to be non-emergency treatment.
  • Access to B12 injections – Although some people received injections, either as normal or at a different GP practice, we also heard that in many areas there was an inconsistent approach to providing this treatment.
  • Care homes – People’s feedback highlighted that while family and friends were unable to visit their loved ones in care homes, timely and regular communication from care home staff really mattered.
  • The hidden effect on families and carers – The lack of respite has left many carers feeling stressed, isolated and forgotten about. We also heard about the difficulties some people faced helping the person that they care for to understand and remember the lockdown measures.
  • Praise for health and social care workers – Throughout the pandemic, we have heard about how much people appreciate the hard work of health and social care professionals during this time of unprecedented challenges.
  • The impact on people’s mental health – Since the start of lockdown, we have heard about the effects of the pandemic on people’s mental health and wellbeing. For some people, the changes to the services they would usually access have left them feeling abandoned – with infrequent telephone appointments not meeting their needs.

What can services learn?

  • Accessible information and meeting people’s communication needs must be considered from the start and should not be an afterthought. The information must also be shared through trusted sources, such as community centres and groups.
  • Families and carers have been providing even more care than usual during the pandemic – but this often goes unnoticed, and many need more support.
  • Good communications between care homes staff residents and their family and friends is key, especially while visiting restrictions are in place. Where appropriate, this should include involving residents’ families or next of kin in decision making about their care.
  • The mental health impacts of the pandemic are affecting both existing service users and non-service users. Mental health services will require investment to support people in both the short and long-term.

Digital healthcare

The pandemic has seen the digitisation of many health and social care services overnight. While digital appointments don’t work for everyone, and services should not be exclusively digital, it’s important healthcare services embrace technology for those who find it an efficient way to communicate. Public Coronavirus Question Time

Public Coronavirus Question Time

Health and council leaders in Northumberland are to host their first ever online coronavirus public question time.
While case numbers have steadied, they’re still too high in the county, and following the recent introduction of a national lockdownleaders will be answering residents’ questions on Wednesday 18 November, at 5pm.
The panel for Wednesday’s broadcast will include:
  • Cllr Glen Sanderson, Leader of the Council
  • Liz Morgan , Director of Public Health
  • Daljit Lally, Chief Executive
The broadcast, the first of its kind in the region, will look at the current Covid-19 picture in Northumberland, what the national lockdown means for the county, and answer queries or concerns people have.
People can submit their questions to the council via a simple online form 
While council and health bosses might not be able to answer every question, the aim is for the public broadcast to become a regular event and cover the most commonly-asked issues.
Residents will be able to watch the broadcast live on the council’s Facebook page 
Council Leader Glen Sanderson said: “We know the impact this pandemic has had on all our lives and the hard work everyone has put in to stay safe and drive numbers down.
“People have already made huge sacrifices, and many feel exhausted by the strain that it has caused. We have a duty to support our communities to continue all their hard work to get infections under control, and we’re in a good position to build on that.
“The situation is changing almost daily so this is a great opportunity to find out the very latest information from a number of experts, as well as hearing about some of the questions that affect you and your communities – whether you work in, live in or visit our county.
We know this public question time approach has been well-received in other parts of the country and we’d really encourage people to let us know questions they might have which we can address during our first broadcast.”

Our Review of the year and AGM will take place online on Wednesday 21 October, from 2.00pm and 4.00pm. Find out about our work and how we made a difference to people in Northumberland over the last 12 months.

As the Secretary of State for Health and Social Care says ‘People should have phone or video consultations with their doctors unless there is a clinical reason not to’, and that there had been a ‘hugely positive’ response to virtual appointments during the coronavirus pandemic, we will also have guest presenters talking about technology in NHS and social care services. We’ll explore how the coronavirus crisis has accelerated the move to online appointments and consultations, what has worked well and not so well, and what we can expect in the future.

Whilst some of our forums are on specific issues and services, this one is a chance to tell us about any health or social care service you’ve used in the last 12 months. That could be hospitals, GPs, care homes, NHS 111, mental health services, maternity services or dentists and pharmacies.
